When entering "All Other Names used by the Debtor in the last 8 years (include married, maiden and trade names)", it is important to include first and middle names in this field.
If you only enter the last name, your software will not automatically fill in the first and middle names in the appropriate field in ECF as shown in the first example below.
For Example,
