Employee Income Records (Payment Advices)

HIGHLIGHTS

As part of the new Bankruptcy Abuse Prevention and Consumer Protection Act:

Rule 1007(b)(1)(E) copies of all payment advices or other evidence of payment, if any, with all but the last four digits of the debtor’s social security number redacted, received by the debtor from an employer within 60 days before the filing of the petition

We have created a Payment Advices Cover Sheet to help you. The cover sheet helps clarify by providing a check box when:

the debtor has had no income
payment advices are not attached; i.e. self-employed

Reminder: Redact all but the last four digits of the debtor's social security number

The Clerk's office will scan the necessary documents into our system if counsel is unable to do so. Please mail them to U.S. Bankruptcy Court Clerk.
