Rule 9004. General Requirements of Form
- Legibility
- All petitions, pleadings, schedules and other papers shall be clearly legible.
- Caption
- Each paper filed shall contain a caption setting forth the name of the court, the title of the case, the bankruptcy docket number, and a brief designation of the character of the paper.
With the growing number of ECF filers utilizing scanners, it is important to insure that the quality of your scanned document is legible.
It is important when filing documents electronically that this rule be followed. If the pleading needs to be printed for any reason, the caption will play a major role in identifying the document.
Rule 1005. Caption of Petition
The caption of a petition commencing a case under the Code shall contain the name of the court, the title of the case, and the docket number. The title of the case shall include the following information about the debtor: name, employer identification number, last four digits of the social security number, any other tax identification number, and all other names used within six years before filing the petition. If the petition is not filed by the debtor, it shall include all names used by the debtor which are known to petitioners
